A bequest to Middlesex can be made by creating a new will, adding a codicil to your present will, or including Middlesex in your revocable trust.

Unrestricted Bequest "I give (all the rest, residue and remainder of my estate) or (______ dollars) to Middlesex School, 1400 Lowell Road, Concord, MA 01742, for the benefit of ________________."
Bequest to Establish an Endowment
"I give (all the rest, residue and remainder of my estate) or (________ dollars) to Middlesex School, 1400 Lowell Road, Concord, MA 01742, and the income only to be used for the following purpose: (state the purpose). If, in the future, it is the opinion of the Head of School and Trustee's, that all or part of the income of this fund cannot be usefully applied to such a purpose, the Head of School and Trustee's, may use the same for any purpose that will most nearly accomplish my wishes."
